Questionnaire completion and acknowledgement
The completion fields record completion, supervision and respondent acknowledgement. Separate these administrative fields from the interview consent process and from the verification of particular answers.

Design use
Record each signatory’s role and the date of completion. Keep the respondent’s acknowledgement distinct from a supervisor’s review, and provide a way to document corrections made after either step.
